Close #34: Improve Mac OSX installation

Update very outdated INSTALL.mac file.
Accept a missing code-signing key while creating xca.app

For 1) install the qt-mac-carbon-opensource-4.x.y.dmg

- create an empty file "build-libs"
- execute xca-2.0.1/misc/build-mac.sh
